office of each state you were ever licensed in. Some states use
DMV or DPS as the acronym for their license issuing authority.
Each state has slightly different guidelines and requirements
for requesting your record, so it is best to call or e-mail them
first. The cost for the record is shown (cost subject to change
by the respective DPS / DMV / MVR / DOT / MVD offices).
